Fat only, or fat plus skin and muscle

Neck liposuction removes fat from under the chin through small incisions. It does not tighten skin or muscle. A neck lift goes further. It removes or repositions fat, tightens the platysma muscle, trims loose skin and, when needed, reduces the glands under the jaw. Liposuction reshapes. A neck lift rebuilds.

When liposuction is enough

Liposuction works when the problem is fat and your skin still snaps back. Younger patients with good elasticity and a defined jawline often get a clean result from lipo alone, because the skin tightens on its own after the fat is gone. If your skin is loose or you have muscle bands, lipo alone will not fix it.

When you need a neck lift

Choose a neck lift if you have loose or crepey skin under the chin, vertical platysma bands, or a heavy neck that no longer matches your face. Those problems come from skin and muscle, not just fat, so they need tightening, not suction alone.

The deep neck question

Most heavy necks need more than skin and fat work. A 2025 review found that about 70 percent of deep neck lifts need some reduction of the submandibular glands, and about 59 percent need modification of the digastric muscles. These deeper structures create the neck angle. Dr. Franco addresses them when they cause a rounded or heavy neck, which is why his results look natural from every angle.

If you have had non-surgical treatments before

Prior neck treatments can change the plan. A 2025 study found that 94 percent of 180 patients who had earlier non-surgical neck treatments, like Kybella, Morpheus8 or Ultherapy, needed a full deep cervicoplasty. Scar tissue from those treatments makes liposuction alone less effective, and most of those patients benefit from a proper neck lift.

Cost

In Medellín, a neck lift alone runs from about $4,000 to $5,500, a fraction of the $12,000 to $20,000 it often costs in the United States. Neck liposuction is quoted by range and generally costs less, but it is only an option for a narrow group of patients. Your written quote depends on what your neck actually needs.
